      目的 探讨新发2型糖尿病(T2DM)的血清网膜素1(Omentin-1)及其基因Val109Asp多态性位点与肠道梭菌属和乳杆菌属的相关性。方法 选取2020年5月至2022年6月在新疆医科大学第一附属医院和第五附属医院就诊的98例新发2型糖尿病(T2DM)患者和同期来院体检的99例糖耐量正常(NGT)健康人作为研究对象,分为T2DM组和NGT组,利用一般资料问卷收集研究对象的年龄、性别、身高和体质量等一般资料;用酶联免疫吸附法(ELISA)测血清Omentin-1。使用限制性核酸内切酶对Omentin-1基因多态性位点Val109Asp进行基因分型。采用16S rRNA实时荧光定量聚合酶链式反应(RT-qPCR)定量检测粪样中梭菌属和乳杆菌属的相对丰度水平;分析梭菌属和乳杆菌属与空腹血糖、血脂、Omentin-1及其基因多态性位点Val109Asp的相关性,探讨肠道菌群和Omentin-1及其两者交互作用对新发T2DM影响。结果 T2DM组血清Omentin-1水平低于NGT组,差异有统计学意义(P<0.05)。T2DM组和NGT组的Omentin-1基因Val109Asp多态性位点分析结果显示差异无统计学意义(P>0.05);根据Pearson相关分析,梭菌属相对[丰度与身体质量指数(BMI)、腰臀比(WHR)、空腹血糖(FPG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)皆呈负相关(r=-0.172、-0.183、-0.455、-0.238、-0.143,P<0.05),与收缩压(SBP)、乳杆菌属水平、Omentin-1水平呈正相关(r=0.325、0.381、0.166,P均<0.05);logistic回归分析显示,血清胰岛素水平、LDL-C、乳杆菌属和梭菌属丰度是新发T2DM的影响因素(P<0.05)。结论 维吾尔族新发T2DM患者血清Omentin-1水平、肠道乳杆菌属和梭菌属的相对丰度发生变化,梭菌属的相对丰度与血清Omentin-1水平正相关。

      Objective To study the correlation between omentin-1, omentin-1 gene polymorphism Val109Asp and abundant of the Clostridium, Lactobacillus genus in newly diagnosed T2DM. Methods A total of 98 Uyghur newly diagnosed T2DM patients and 99 subjects with normal glucose tolerance were enrolled from First and Fifth Affiliated Hospital from May 2020 to June 2022, then all subjects were divided into T2DM and NGT group. The general information questionnaire was used to collect the general information such as age, gender, height and weight, etc. The level of serum omentin-1 was detected by enzyme linked immunosorbent assay. The genotype of Val109Asp polymorphism in the omentin-1 gene was determined by PCR-PFLP. Abundant of the Clostridium and Lactobacillus genus were determined with 16S rRNA gene real-time fluorescence quantitative PCR amplification. Besides, the relationship between fasting plasma glucose (FPG), blood lipids, omentin-1, Val109Asp polymorphism, abundance of Clostridium genus and abundance of Lactobacillus genus was assessed to investigate the effects of intestinal microbiota and omentin-1 and their interactions on newly diagnosed T2DM. Results Serum omentin-1 level in T2DM group was lower than that in NGT group, and the difference was statistically significant (P<0.05). There were no significant differences between the genotype of Val109Asp variant in two groups (P>0.05). According to Pearson correlation analysis, the abundance of Clostridium was negatively correlated with BMI, WHR, FPG, total cholesterol (TC), low density lipoprotein-cholesterol (LDL-C) (r=-0.172,-0.183,-0.455,-0.238,-0.143,all P<0.05) and positively correlated with systolic blood pressure (SBP), abundance of Lactobacillus genus and Omentin-1 level (r=0.325,0.381,0.166,all P<0.05). Logistic regression analysis found that serum insulin, LDL-C, the abundances of Lactobacillus genus and Clostridium genus were the risk factors of newly diagnosed T2DM (all P<0.05). Conclusions The serum omentin-1 level, the relative abundance of Lactobacillus and Clostridium are changed in Uyghur newly diagnosed T2DM patients, and the relative abundance of Clostridium genus is positively correlated with serum Omentin-1 level.
