      目的 探讨甲状腺乳头状癌(PTC)患者环状RNA_0015278(circ_0015278)表达与甲状腺癌及预后的相关性。方法 回顾性分析2016年6月至2021年12月承德医学院附属医院收治的甲状腺切除术后PTC患者206例,通过逆转录定量聚合酶链反应(RT-qPCR)检测PTC及癌旁组织中circ_0015278的表达,以病理结果为金标准,使用受试者工作特征(ROC)曲线评价circ_0015278在PTC及癌旁组织中的诊断价值;circ_0015278表达按照分位数划分四等级,使用Spearman相关分析,分析不同等级分位数circ_0015278表达与PTC 5年复发率、死亡率的相关性。使用KM曲线对数秩检验对不同等级分位数circ_0015278表达患者随访5年的无病生存率(DFS)和总生存率(OS)进行比较。通过单变量和多变量Cox比例风险回归模型分析影响PTC患者预后的因素。结果 PTC组织中circ_0015278表达与癌旁组织相比明显降低,差异有统计学意义(Z=-14.146,P< 0.001);以病理结果为金标准建立PTC及癌旁组织circ_001527表达的ROC曲线,以表达量0.740为截断值时对鉴别PTC及癌旁组织有较好价值(AUC:0.903,95% CI:0.847~0.932),灵敏度为76.7%,特异度为89.2%;较高的circ_0015278表达与肿瘤复发率降低相关(r=-0.162,P=0.011),与死亡率无关(r=-0.102,P=0.110);不同等级分位数circ_0015278表达的DFS之间差异有统计学意义(χ2=3.268,P=0.017),利于DFS延长。OS之间差异无统计学意义(χ2=5.551,P=0.136);多因素Cox回归分析表明,较高肿瘤circ_0015278表达与有利的DFS独立相关(HR=0.529,P=0.026)。结论 PTC组织中circ_0015278表达明显降低,对PTC有较好的诊断价值,其高表达与肿瘤复发率降低及患者DFS延长相关。

      Objective To investigate the correlation between the expression of circular RNA_0015278(circ_0015278) and thyroid cancer and prognosis in patients with papillary thyroid carcinoma (PTC). Methods A retrospective analysis was performed on 206 PTC patients who underwent thyroidectomy in the Affiliated Hospital of Chengde Medical College from June 2016 to December 2021. The expression of circ_ 0015278 in PTC and adjacent tissues was detected by reverse transcription quantitative polymerase chain reaction (RT-qPCR). The diagnostic value of circ_0015278 in PTC and adjacent tissues was evaluated using receiver operating characteristic (ROC) curve. The expression of circ_ 0015278 was divided into four levels according to the quantile. The correlation between circ_0015278 expression in different quantiles and the 5-year recurrence rate and mortality rate of PTC was analyzed by Spearman correlation analysis. The 5-year disease-free survival (DFS) and overall survival (OS) of patients with circ_0015278 expression at different quantiles were compared in KM curve log-rank test. The prognostic factors of PTC patients were assessed by univariate and multivariate Cox proportional hazards regression models. Results The expression of circ_0015278 in PTC tissues was significantly lower than that in adjacent tissues (Z=-14.146, P< 0.001). The ROC curve of circ_001527 expression in PTC and adjacent tissues was established using pathological results as the gold standard. The cut-off value of 0.740 was better for the identification of PTC and adjacent tissues (AUC: 0.903, 95%CI: 0.847~0.932), with a sensitivity of 76.7% and a specificity of 89.2%. Higher circ_0015278 expression was associated with decreased tumor recurrence rate (r=-0.162, P=0.011), but not with mortality rate (r=-0.102, P=0.110). The DFS with different quantiles of circ_0015278 expression showed statistically significant difference (χ2=3.268, P=0.017), which was conducive to the extension of DFS. There was no significant difference between OS (χ2=5.551, P=0.136). Multivariate Cox regression analysis showed that higher tumor circ_0015278 expression was independently associated with favorable DFS (HR=0.529, P=0.026). Conclusions The expression of circ_0015278 in PTC tissues significantly decreases, which has good diagnostic value for PTC. The high expression is associated with the decrease of tumor recurrence rate and the prolongation of DFS in patients.
