      目的 探讨早产儿支气管肺发育不良(BPD)的危险因素及血浆骨形成蛋白-7(BMP-7)、微小核糖核酸-15b (miR-15b)、25羟基维生素D3[25-(OH)D3]对其诊断价值。方法 回顾性分析2022年1月至2023年3月河北省沧州中西医结合医院收治的136例早产儿的临床资料,根据其是否伴有BPD,将其分为BPD组(n=45)和非BPD组(n=91)。分析早产儿BPD的单因素,采用多因素logistic回归分析早产儿BPD的危险因素,绘制受试者工作特征(ROC)曲线分析血浆BMP-7、miR-15b、25-(OH)D3对早产儿BPD的诊断价值。结果 两组患儿机械通气时间、吸氧时间、有无脓毒血症及血浆BMP-7、miR-15b、25-(OH)D3水平比较,差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,机械通气时间长、血浆BMP-7、miR-15b水平偏高、血浆25-(OH)D3水平偏低是早产儿BPD的独立危险因素(OR=2.625、2.208、2.280、2.517,P<0.05)。血浆BMP-7、miR-15b、25-(OH)D3联合诊断早产儿BPD的曲线下面积为0.947,高于血浆BMP-7、miR-15b、25-(OH)D3单独检测(0.822、0.849、0.824,P<0.05)。结论 早产儿BPD的危险因素与机械通气时间长、血浆BMP-7、miR-15b水平偏高、血浆25-(OH)D3水平偏低有关,且血浆BMP-7、miR-15b、25-(OH)D3联合检测对早产儿BPD的诊断价值更高。

      Objective To investigate the risk factors of bronchopulmonary dysplasia (BPD) in premature infants and the diagnostic value of plasma bone morphogenetic protein-7 (BMP-7),microribonucleic acid-15b (miR-15b),25 hydroxyvitamin D3[25-(OH)D3].Methods The clinical data of 136 preterm infants who were admitted to Cangzhou Integrated Traditional Chinese and Western Medicine Hospital from 2022 to 2023 were extracted for the study.The infants were divided into the BPD group (n=45) and the non-BPD group (n=91) based on whether they were accompanied by BPD.The univariate and multivariate logistic regression were used to identify the risk factors of BPD in preterm infants.The receiver operating characteristic (ROC) curve was plotted to assess the diagnostic value of BMP-7,miR-15b and 25-(OH)D3 for BPD in preterm infants.Results There were significant differences in mechanical ventilation time,oxygen inhalation time,sepsis and levels of plasma BMP-7,miR-15b and 25-(OH)D3 between the two groups (P<0.05).Multivariate logistic regression analysis showed that long duration of mechanical ventilation,high levels of plasma BMP-7 and miR-15b and low level of plasma 25-(OH)D3 were independent risk factors for BPD in premature infants (OR=2.625,2.208,2.280,2.517,P<0.05).The area under the curve value of plasma BMP-7,miR-15b and 25-(OH)D3 combined for diagnosis of BPD in premature infants was 0.947,which was higher than that of plasma BMP-7,miR-15b and 25-(OH)D3 alone (0.822,0.849,0.824,P<0.05).Conclusion The risk factors of BPD in preterm infants are associated with long mechanical ventilation time,high levels of plasma BMP-7 and miR-15b and low level of plasma 25-(OH)D3,and the combined detection of plasma BMP-7,miR-15b and 25-(OH)D3 has higher diagnostic value for BPD in preterm infants.
