目的 探究急性脑梗死(ACI)患者血清miR-149-5p表达水平与患者病情及预后的关系。方法 选取2019年3月至2020年3月保定市第一中心医院治疗的124例确诊的ACI患者为患病组,根据病情分为重度组(36例)、中度组(46例)和轻度组(42例),根据随访结果分为预后不良组(40例)和预后良好组(84例);另选取同期来医院体检的124例健康体检者为对照组。采用荧光定量聚合酶链式反应(PCR)法测定血清中miR-149-5p的表达水平;采用Pearson法分析血清miR-149-5p水平与病情的相关性;logistic回归分析患者预后的影响因素;受试者工作特征(ROC)曲线分析血清miR-149-5p水平对患者预后不良的诊断价值。结果 患病组患者血清miR-149-5p水平为(0.90±0.29),低于对照组(P<0.05);随着病情严重程度上升,miR-149-5p水平呈下降趋势(P<0.05);预后不良组患者血清miR-149-5p水平低于预后良好组(P<0.05);年龄≥70岁、脑梗死部位为脑干、大面积脑梗死是患者预后不良的危险因素,miR-149-5p升高是患者预后不良的保护因素(P<0.05);ROC曲线结果显示,血清miR-149-5p诊断ACI患者预后不良的曲线下面积为0.873,截断值为0.768,灵敏度为85.00%,特异度为76.19%。结论 ACI患者血清miR-149-5p水平下调,患者病情恶化,miR-149-5p为预后的保护因素,miR-149-5p对ACI患者预后不良具有一定的诊断价值。 |

Objective To explore the relationship between serum miR-149-5p expression levels and the condition and prognosis of patients with acute cerebral infarction (ACI). Methods From March 2019 to March 2020, 124 confirmed ACI patients treated in Baoding No.1 Central Hospital were selected as the research subjects,according to the severity of the condition, they were grouped into the mild group (42 cases), moderate group (46 cases), and severe group (36 cases);according to the follow-up results, all patients were divided into a poor prognosis group (40 cases) and a good prognosis group (84 cases);124 healthy individuals who came to the hospital for physical examination during the same period were selected as the control group.Fluorescence quantitative Polymerase Chain Reaction (PCR) method was applied to measure the expression level of miR-149-5p in serum;Pearson method was applied to analyze the correlation between serum miR-149-5p level and the severity of the disease;logistic regression analysis was performed to analyze the influencing factors of patient prognosis. The ROC curve was used to analyze the diagnostic value of serum miR-149-5p level in poor prognosis. Results The serum miR-149-5p level in the diseased group was (0.90 ±0.29), which was lower than that in the control group (P<0.05);as the severity of the disease increases, the level of miR-149- 5p showed a decreasing trend (P<0.05);the serum miR-149-5p level in the poor prognosis group (0.52±0.17) was obviously lower than that in the good prognosis group (1.08±0.35) (P<0.05);age ≥ 70 years old, location of cerebral infarction in the brainstem and extensive cerebral infarction were the risk factors that affected the prognosis of patients with ACI, while elevated miR-149-5p was a protective factor that affected the prognosis of patients with ACI (P<0.05);the ROC curve results showed that the area under the curve for diagnosing poor prognosis in ACI patients with serum miR-149-5p was 0.873, the cutoff value was 0.768, the sensitivity was 85.00%, and the specificity was 76.19%. Conclusion The serum miR-149-5p level is down-regulated in patients’ with ACI, and the patient′s condition deteriorates. MiR-149-5p is a protective factor for prognosis, and miR-149-5p has certain diagnostic value in poor prognosis in patients with ACI. |
