      目的 观察并对比浓缩生长因子(CGF)与胶原塞应用于下颌低位阻生第三磨牙拔除的临床效果。方法 选取 2023年 2 月至 2024 年 3 月在合肥市口腔医院口腔颌面外科门诊因出现明显临床症状或正畸治疗方案需要而拔除双侧下颌低位阻生第三磨牙的 48 例患者,采用自身双侧对照临床试验设计,患者先后分次拔除双侧下颌阻生第三磨牙,为促进创口愈合,拔牙窝分别填充CGF 凝胶与胶原塞。观察不同填充物患者术后 30 分钟及 24 小时出血情况以及在第 1、3、7 天的疼痛、肿胀、张口受限程度及干槽症发生情况等差异。结果 采用 CGF 和胶原塞填充的患者手术时间、出血情况、干槽症发生情况比较,差异均无统计学意义(P>0.05);两组患者术后开口度、疼痛评分及肿胀程度存在时间效应和组间效应(P<0.05)。进一步比较,CGF 组在术后第 1、3 天的开口度大于胶原组,疼痛评分及肿胀程度小于胶原组,差异有统计学意义(P<0.05);在术后第 7 天,CGF 组和胶原组比较,开口度、疼痛评分及肿胀程度差异均无统计学意义(P>0.05)。结论 相比填充胶原塞,填充 CGF 凝胶能更有效地改善下颌阻生第三磨牙拔牙患者术后早期疼痛、面部肿胀及张口受限。

      Objective To observe the clinical effect of concentrated growth factor application in the extraction of mandibular low impacted third molars while compared with collagen plug. Methods From Feburary 2023 to March 2024, 48 patients with bilateral mandibular impacted third molars in the Outpatient Department of Oral and Maxillofacial Surgery, Hefei Stomatological Hospital were selected.These patients were chosen based on obvious clinical symptoms or as part of an orthodontic treatment plan. A self-controlled clinical trial design was applied in this study.The patient had the bilateral impacted third molar extracted successively. In order to promote wound healing, the extraction sockets were filled with CGF gel and collagen plugs, respectively. The postoperative bleeding at 30 minutes and 24 hours, pain, swelling, limitation of mouth opening and dry socket on the 1st, 3rd and 7th day after operation were observed. Results There was no significant difference in surgical time, bleeding, and dry socket syndrome between the CGF and collagen plug groups (P>0.05). However, there were significant time effects and inter-group effects observed in postoperative mouth opening, VAS pain score, and swelling degree between the two groups (P < 0.05). Further analysis revealed that on the first and third day after surgery, the CGF group had a higher degree of mouth opening than the collagen group while also experiencing lower VAS pain scores and less swelling; these differences were statistically significant (P < 0.05). There was no significant difference in the degree of mouth opening, VAS pain score and swelling between the CGF group and the collagen group on the 7th day after operation (P>0.05). Conclusions Compared with collagen plug filling, CGF gel is more effective in improving early postoperative pain, facial swelling, and mouth opening in patients with impacted mandibular third molar extraction.
