| 目的 旨在评估广泛期小细胞肺癌(ED-SCLC)患者代谢水平与标准一线治疗的相关性。材料与方法 回顾性分析2020年12月至2023年12月在南京大学医学院附属鼓楼医院呼吸科接受标准一线治疗的SCLC患者的数据。评估体重指数(BMI)、血清白蛋白和血脂水平及其较基线变化值(△)与疗效的相关性。结果 在90例接受标准一线治疗的SCLC患者中,与有效组(CR+PR+SD组)相比,PD组血清白蛋白及载脂蛋白AI水平较低(P<0.01),而NSE、CEA及CYFRA 211水平较高(P<0.001)。多变量分析发现低水平NSE与长PFS显著相关(p=0.008)。同时分析各指标较基线变化值(△)对疗效的影响。与有效组相比,PD组患者的△血清白蛋白、△载脂蛋白AI水平下降更明显(P<0.05),而△NSE、△CEA及△CYFRA 211水平升高更明显(P<0.05)。多变量分析发现△甘油三酯和△载脂蛋白AI水平升高与较长的PFS显著相关,p值分别为0.005和0.006。结论 低水平的NSE、高水平的载脂蛋白AI及△载脂蛋白AI与PFS延长相关,有助于预测标准一线疗效。 |

| Objective To evaluate the correlation between metabolic level and standard first-line treatment in patients with extensive stage small cell lung cancer (ED-SCLC). Materials and methods The data of SCLC patients who received standard first-line treatment in the respiratory department of Gulou Hospital Affiliated to Nanjing University School of medicine from December 2020 to December 2023 were retrospectively analyzed. Objective to evaluate the correlation between body mass index (BMI), serum albumin and lipid levels and their changes from baseline (△) and curative effect. Results Among the 90 SCLC patients who received standard first-line treatment, compared with the effective group (cr+pr+sd group), the levels of serum albumin and apolipoprotein AI in the PD group were lower (p<0.01), while the levels of NSE, CEA and CYFRA 211 were higher (p<0.001). Multivariate analysis showed that low-level NSE was significantly associated with long PFS (P = 0.008). At the same time, the influence of the change (△) of each index from baseline on the curative effect was analyzed. Compared with the effective group, the levels of △serum albumin and △apolipoprotein AI in the PD group decreased more significantly (p<0.05), while the levels of △ NSE, △CEA and △CYFRA 211 increased more significantly (p<0.05). Multivariate analysis showed that elevated levels of △triglyceride and △apolipoprotein AI were significantly associated with longer PFS, with P values of 0.005 and 0.006, respectively. Conclusion Low level of NSE, high level of apolipoprotein AI and △apolipoprotein AI are associated with prolonged PFS, which is helpful to predict the effect of standard first-line treatment. |
