| 目的 探讨哮喘患儿血清CXC趋化因子受体3(CXCR3)、黏蛋白5AC(MUC5AC)表达水平及其对预后的预测价值。方法 选取2020年3月至2023年3月开滦总医院林西医院收治的138例哮喘患儿为哮喘组,另选择本院同期138例体检儿童为对照组。随访1年,根据哮喘控制测试表评分结果,将哮喘患儿分为预后不良组(n=41)和预后良好组(n=97)。血清CXCR3、MUC5AC表达水平采用酶联免疫吸附测定(ELISA)法测定;哮喘患儿预后的影响因素采用多因素logistic回归分析;血清CXCR3、MUC5AC对哮喘患儿预后的预测价值绘制受试者工作特征(ROC)曲线分析。结果 与对照组相比,哮喘组血清CXCR3、MUC5AC表达水平升高(P<0.05)。预后良好组相比,预后不良组血清CXCR3、MUC5AC表达水平升高(P<0.05)。Logistic回归分析结果显示,CXCR3(OR=1.375)、MUC5AC(OR=2.134)、疾病严重程度(OR=1.587)、家族哮喘史(OR=1.694)是影响哮喘患儿预后的危险因素(P<0.05)。ROC曲线结果显示,血清CXCR3、MUC5AC二者联合预测哮喘患儿预后的曲线下面积最高,优于血清CXCR3、MUC5AC各自单独预测(Z二者联合-CXCR3=3.979,P<0.001;Z二者联合-MUC5AC=3.279,P=0.001)。结论 哮喘患儿血清CXCR3、MUC5AC水平升高,二者联合可以更准确地预测哮喘患儿预后,具有较高的预测价值。 |

| Objective To explore the expression levels of serum CXC chemokine receptor 3 (CXCR3) and mucin 5AC (MUC5AC) in children with asthma and their predictive value for prognosis.Methods A total of 138 children with asthma admitted to Linxi Hospital of Kailuan General Hospital from March 2020 to March 2023 were selected as the asthma group, and 138 healthy children were selected as the control group. Follow-up for one year, according to the score of asthma control test table, children with asthma were divided into the poor prognosis group (n=41) and good prognosis group (n=97). The expression levels of serum CXCR3 and MUC5AC were determined by enzyme-linked immunosorbent assay (ELISA). The factors influencing the prognosis of children with asthma were analyzed by multivariate logistic regression. The predictive value of serum CXCR3 and MUC5AC on the prognosis of children with asthma was analyzed by drawing ROC curve.Results Compared with the control group, the expression levels of serum CXCR3 and MUC5AC in the asthma group obviously increased (P<0.05). The expression levels of serum CXCR3 and MUC5AC in the poor prognosis group were significantly higher than those in the good prognosis group (P<0.05). Multivariate logistic regression found that CXCR3 (OR=1.375), MUC5AC (OR=2.134), disease severity (OR=1.587), and family history of asthma (OR=1.694) were the risk factors affecting the prognosis of children with asthma (P<0.05). The ROC curve showed that the combined prediction of serum CXCR3 and MUC5AC had the highest area under the curve for prognosis in children with asthma, which was better than the individual prediction of serum CXCR3 and MUC5AC (Zcombination-CXCR3=3.979, P<0.001; Zcombination-MUC5AC=3.279, P=0.001).Conclusion The expression levels of serum CXCR3 and MUC5AC in children with asthma increase. The combination of them can predict the prognosis of children with asthma more accurately and has high predictive value. |
