| Objective To discuss the relationship between serum miR-455, miR-125a-5p with oxidative stress and disease severity in patients with acute cerebral infarction (ACI). Methods Between April 2022 and June 2024, 98 ACI patients with ACI admitted to Qin huangdao Hospital of Traditional Chinese Medicine were included. They were assigned into the mild group (43 cases), moderate group (21 cases), and severe group (34 cases) complying with disease severity, and 60 healthy medical check-ups in the same period were selected as the control group. Real-time fluorescence quantitative reverse transcription-polymerase chain reaction (qRT-PCR) was employed to assay serum miR-455 and miR-125a-5p. Pearson correlation analysis was used to analyze the correlation between serum miR-455, miR-125a-5p and oxidative stress indicators in ACI patients. Logistic regression model was performed to analyze the factors affecting the severity of ACI patients. Moreover, receiver operating characteristic (ROC) curve was used to evaluate the diagnostic value of serum miR-455 and miR-125a-5p for the severity of ACI patients. Results Serum miR-455 and miR-125a-5p levels were lower in the ACI group than in the control group (P<0.05). The severe group hadhigher stroke volume, National Institutes of Health Stroke Scale (NIHSS) score, and malondialdehyde (MDA), and lower serum superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), miR-455, and miR-125a-5p than mild and moderate groups, and the differences between mild and moderate groups were statistically significant (P<0.05). Serum miR-455 and miR-125a-5p were positively correlated with SOD and GSH-Px, and negatively correlated with MDA (P<0.05). The infarct volume and serum SOD, MDA, GSH-Px, miR-455, and miR-125a-5p were influencing factors on the severity of ACI patients (P<0.05). The area under the curve (AUC) of the joint diagnosis of serum miR-455 and miR-125a-5p for the severity of ACI patients was 0.920, and their diagnostic value was higher than their respective diagnoses (Zjoint-miR-455=2.063, Zjoint-miR-125a-5p=2.259, P<0.05). Conclusion Serum miR-455 and miR-125a-5p in ACI patients are reduced and closely related to oxidative stress and disease severity. The joint of the two has higher diagnostic value for the severity of ACI patients. |
