| 目的 探讨子痫前期(PE)高风险患者早期干预后的可溶性fms样酪氨酸激酶-1(SFLT-1)、胎盘生长因子(PLGF)表达水平及其对不良妊娠结局的预测价值。方法 回顾性分析皖北煤电集团总医院2021年11月至2024年5月收治的111例PE高风险患者资料,根据最终是否发生不良妊娠结局分为发生组(n=63)、未发生组(n=48),并选取本院同期30例健康产妇作为对照组(n=30)。对比3组产妇SFLT-1、PLGF、SFLT-1/PLGF水平。使用logistic多因素分析PE高风险患者干预后发生不良妊娠结局的危险因素,采用受试者工作特征(ROC)曲线分析SFLT-1、PLGF、SFLT-1/PLGF及3者联合对PE高风险患者早期干预后不良妊娠结局的预测价值。结果 与对照组和未发生组比较,发生组血清SFLT-1、SFLT-1/PLGF水平升高,而PLGF水平降低(P<0.05)。多因素logistic回归分析结果显示,SFLT-1(OR=1.002)、PLGF(OR=0.902)、SFLT-1/PLGF(OR=1.053)是PE高风险患者不良妊娠结局的影响因素(P<0.05)。血清SFLT-1、PLGF、SFLT-1/PLGF单独预测PE高风险患者不良妊娠结局的曲线下面积(AUC)分别为0.960、0.917、0.875,3者联合预测的AUC为0.990,高于单独预测的AUC(P<0.05)。Hosmer-Lemeshow分析结果显示,χ2=4.091,P=0.849。结论 PE高风险患者早期干预后的血清SFLT-1、PLGF及SFLT-1/PLGF水平与不良妊娠结局有关,且3指标联合可更准确地预测PE高风险患者的妊娠结局。 |

| Objective To explore the expression levels of soluble FMS-like tyrosine kinase-1(SFLT-1), placental growth factor(PLGF), and their ratio(SFLT-1/PLGF) in patients at high risk of preeclampsia(PE) after early intervention, and their predictive value for adverse pregnancy outcomes. Methods A retrospective analysis was conducted on the data of 111 patients at high risk of PE admitted to the General Hospital of Wanbei Coal and Electricity Group from November 2021 to May 2024. They were divided into the occurrence group(n=63) and the non-occurrence group(n=48) based on whether adverse pregnancy outcomes occurred in the end. Additionally, 30 healthy parturients from our hospital during the same period were selected as the control group(n=30). The levels of maternal SFLT-1, PLGF and SFLT-1/PLGF in the three groups were compared, and the risk factors of adverse pregnancy outcomes in high-risk PE patients after early intervention were analyzed by logistic multivariate analysis. Receiver operating characteristic(ROC) curve analysis The predictive value of SFLT-1, PLGF, SFLT-1/PLGF ratio, and their combination in predicting adverse pregnancy outcomes in preeclampsia(PE) high-risk patients after early intervention was analyzed using receiver operating characteristic(ROC) curve analysis. Results Compared with the control group and the nonoccurrence group, the levels of serum SFLT-1 and SFLT-1/PLGF in the occurrence group increased, while the level of PLGF decreased(P <0.05). The results of multivariate logistic regression analysis showed that the levels of SFLT-1(OR=1.002), PLGF(OR=0.902), and SFLT-1/PLGF(OR=1.053) were influencing factors affecting adverse pregnancy outcomes in patients at high risk of PE(P <0.05). The area under the curve(AUC) of serum SFLT-1, PLGF, and SFLT-1/PLGF alone for predicting adverse pregnancy outcomes in patients at high risk of PE was 0.960, 0.917, and 0.875, respectively. The AUC predicted by the combination of the three was 0.990, which was higher than the AUC predicted alone(P <0.05). Hosmer-Lemeshow results: χ2=4.091, P=0.849>0.05. Conclusions The levels of serum SFLT-1, PLGF and SFLT-1/PLGF in patients at high risk of PE after early intervention are related to adverse pregnancy outcomes. Moreover, the combination of the three indicators can predict the pregnancy outcomes of patients at high risk of PE more accurately and has a high predictive value. |
