Objective To explore the mediating role of health-promoting behaviors between fear of disease progression and quality of life in postoperative cervical cancer patients, aiming to improve patients' quality of life and provide a basis for developing effective interventions. Methods Using convenience sampling, cervical cancer patients admitted to two tertiary hospitals in Nanchong from January 2025 to March 2025 were selected as the survey subjects, and all patients underwent surgical treatment. The survey was conducted using the General Information Questionnaire, the Fear of Progression Questionnaire-Short Form(FoP-Q-SF), the Health-Promoting Lifestyle Profile-II Revised(HPLP-ⅡR), and the Functional Assessment of Cancer Therapy-Cervix(FACT-Cx). Results A total of 243 questionnaires were collected, with 237 valid responses, yielding an effective response rate of 97.53%. The total score of FoP-Q-SF, HPLP-ⅡR, and FACT-Cx for postoperative cervical cancer patients was(32.22 ± 5.97),(97.12 ± 10.30), and(107.65 ± 15.45), respectively. FoP-Q-SF was negatively correlated with HPLP-ⅡR(r=-0.247, P < 0.001) and FACT-Cx(r=-0.556, P < 0.001), while HPLP-ⅡR was positively correlated with FACT-Cx(r= 0.511, P< 0.001). Health-promoting behaviors partially mediated the effect of fear of disease progression on quality of life(β=-0.138, P< 0.001), accounting for 9.70% of the total effect. Conclusion Fear of disease progression can directly affect quality of life and indirectly influence it through health-promoting behaviors. Nursing staff can take effective measures to reduce patients' fear of disease progression, establish healthy behaviors, and thereby improve their quality of life.
