| 目的 探讨共表达甲状腺转录因子1(TTF-1)与p40蛋白的非小细胞肺癌(NSCLC)临床病理特征、免疫表型、基因改变及其治疗与预后。方法 收集2023年1月至2025年4月安徽医科大学第二附属医院诊断的6例及文献报道的75例(共81例)TTF-1与p40蛋白共表达NSCLC患者,总结81例患者的临床资料、病理特征、免疫组化及基因检测结果,并分析相关数据。结果 共表达TTF-1与p40蛋白的NSCLC肿瘤好发于老年男性(男女比约3∶1),镜下肿瘤细胞常呈实性巢状排列(78/81),仅2例出现局灶性腺样结构(2/81),10例出现局灶性鳞状分化(10/81),细胞异型性大,核分裂像易见,同一区域同时具有腺癌和鳞状细胞癌的免疫表型,弥漫表达TTF-1与p40蛋白(81/81),常出现TP53基因改变(40/61),肺癌常见驱动基因改变并不一致。结论 共表达TTF-1与p40蛋白的NSCLC具有独特的临床病理特征及免疫表型,其预后较差。 |

| Objective To investigate the clinicopathological features, immunophenotype, genetic alterations, and treatment prognosis of non-small cell lung cancer (NSCLC) with co-expression of thyroid transcription factor-1 (TTF-1) and p40 protein. Methods A total of 81 non-small cell lung cancer (NSCLC) cases with co-expression of TTF-1 and p40 protein were collected, including 6 cases diagnosed at the Second Hospital of Anhui Medical University from January 2023 to April 2025 and 75 cases reported in the literature. Clinical data, pathological features, immunohistochemical and genetic testing results were summarized and analyzed. Results This type of tumor predominantly occurred in elderly males (male-to-female ratio was approximately 3∶1). Microscopically, the tumor cells wereare characteristically arranged in solid nested patterns (78/81), with only 2 cases showing focal glandular structures (2/81) and 10 cases exhibiting focal squamous differentiation (10/81). These cells exhibited marked pleomorphism, prominent mitotic activity, and concurrent immunophenotypic features of both adenocarcinoma and squamous cell carcinoma within the same anatomic region. Diffuse immunoreactivity for TTF-1 and p40 iswas consistently identified (81/81), while TP53 gene alterations arewere frequently encountered (40/61); however, alterations in common driver genes of lung cancer arewere not concordant. Conclusion NSCLC with co-expression of TTF-1 and p40 exhibits distinctive clinicopathological features and immunophenotypes, with a relatively poor prognosis. |
