      目的 探究术前血清miR-15a、miR-199b表达对输尿管结石患者术后并发泌尿系统感染的预测价值,并分析术后miRNA表达变化。方法 选取2022年1月至2024年5月汉中市三二〇一医院收治的243例输尿管结石患者为研究对象,根据患者术后是否并发泌尿系统感染分为感染组(n=36)和非感染组(n=207)。采用实时荧光定量聚合酶链反应法(q-PCR)测定各组血清中miR-15a、miR-199b表达水平。利用多因素logistic回归分析术后并发泌尿系统感染的影响因素。绘制受试者工作特征(ROC)曲线分析术前血清miR-15a、miR-199b在输尿管结石患者术后并发泌尿系统感染中的预测价值。结果 与非感染组相比,感染组术前血清miR-15a表达下调(P<0.05),miR-199b表达上调(P<0.05)。泌尿系统感染患者术后血清miR-15a表达低于术前(P<0.05),miR-199b表达高于术前(P<0.05)。合并糖尿病、结石直径≥2 cm、手术时间≥60 min、留置尿管时间≥7 d、血清miR-199b升高是术后并发泌尿系统感染的危险因素(P<0.05),miR-15a升高为其保护因素(P<0.05)。术前血清miR-15a、miR-199b预测输尿管结石患者术后并发泌尿系统感染的曲线下面积(AUC)分别为0.826、0.823,二者联合预测的AUC为0.919,大于miR-15a(Z=2.156,P=0.031)、miR-199b(Z=2.015,P=0.044)单独预测。结论 输尿管结石术后并发泌尿系统感染患者血清miR-15a呈低表达,miR-199b呈高表达,是术后并发泌尿系统感染的影响因素,二者联合具有较高的感染预测价值。

      Objective To explore the predictive value of preoperative serum miR-15a and miR-199b expression for postoperative urinary system infection in patients with ureteral calculi, and to analyze the postoperative miRNA expression changes. Methods This study selected 243 patients with ureteral stones admitted to 3201 Hospital from January 2022 to May 2024 as subjects. According to whether urinary system infections occurred after surgery, the patients were grouped into the infection group(n=36) and non-infection group(n=207). Real-time fluorescence quantitative RCR method(q-PCR) was applied to determine the expression levels of miR-15a and miR-199b in the serum of each group. Multivariate logistic regression was applied to analyze the influencing factors of postoperative urinary system infections. Receiver operating characteristic(ROC) curves were plotted to analyze the predictive value of preoperative serum miR-15a and miR-199b for postoperative urinary system infections in patients with ureteral stones. Results Compared with the non infection group, the preoperative serum miR-15a expression in the infection group was downregulated(P<0.05) and miR-199b expression was upregulated(P<0.05). Compared with that before surgery, the expression of serum miR-15a decreased(P<0.05) and miR-199b increased(P<0.05) in patients with postoperative urinary system infections. Complicated with diabetes, stone diameter ≥ 2 cm, operation time ≥ 60 min, indwelling catheter time ≥ 7 d, and serum elevated miR-199b were the risk factors for postoperative urinary system infection(P<0.05), and elevated miR-15a was the protective factor(P<0.05). The area under the curve predicted by preoperative serum miR-15a and miR-199b for postoperative urinary system infection in patients with ureteral stones was 0.826 and 0.823, respectively. The area under the curve predicted by the combination of the two was 0.919, obviously larger than the area under the curve predicted by miR-15a(Z=2.156, P=0.031) and miR-199b(Z=2.015, P=0.044) alone. Conclusion Serum miR-15a expression is low and miR-199b expression is high in patients with urinary system infections after ureteral stone surgery, they are the factors affecting postoperative urinary system infections. The combination of the two has high predictive value for infections.
