      目的 探讨腔隙性脑梗死合并脑微出血(CMBs)患者血压变异性及脑白质病变特征。方法 选取2024年12月至2025年6月河北中石油中心医院收治的280例腔隙性脑梗死患者为研究对象,其中发生CMBs患者94例纳入观察组,未发生CMBs患者186例纳入对照组,观察两组患者临床一般资料、动态血压参数、脑白质病变评分量表(WMLs)差异,并对观察组不同数量CMBs病灶患者间的差异进行分析。结果 观察组患者年龄及高血压患病比例均高于对照组(P<0.05)。观察组患者的收缩压24 h时平均测定值、舒张压24 h平均测定值、患者夜间收缩压平均测定值、患者在夜间舒张压的平均测定值高于对照组,差异有统计学意义(P<0.05);观察组患者WMLs的Fazekas评分高于对照组,差异有统计学意义(P<0.05)。在CMBs患者中,病灶数≥5个者24 h收缩压变异系数、24 h舒张压变异系数及WMLs评分均高于病灶数<5个者(P<0.05)。结论 腔隙性脑梗死合并CMBs患者血压变异性升高、脑白质病变加重,在不同CMBs病灶数量患者间有明显差异。

      Objective To explore the blood pressure variability and characteristics of white matter lesions in patients with lacunar cerebral infarction complicated with cerebral microbleeds(CMBs). Methods A total of 280 patients with lacunar cerebral infarction were selected, including 94 patients with CMBs(observation group) and 186 patients without CMBs(control group). The differences in clinical general data, dynamic blood pressure parameters, white matter lesion score scales(WMLs) between the two groups were observed, as well as the differences between patients with different numbers of CMBs lesions in the observation group. Results The observation group was older and had a higher prevalence of hypertension compared to the control group(P< 0.05). Regarding blood pressure parameters, the observation group exhibited significantly higher levels in 24-hour mean systolic blood pressure(SBP), 24-hour mean diastolic blood pressure(DBP), nighttime mean SBP, nighttime mean DBP, as well as the 24-hour coefficient of variation(CV) for both SBP and DBP, compared to the control group(P< 0.05). Additionally, the observation group demonstrated significantly higher white matter lesion(WML) scores than the control group(P< 0.05). Among patients with cerebral microbleeds(CMBs), those with ≥5 lesions exhibited higher 24-hour SBP-CV, 24-hour DBP-CV, and WML scores compared to those with <5 lesions(P< 0.05). Conclusion Patients with lacunar cerebral infarction combined with CMBs have increased blood pressure variability and worsened white matter lesions. There is a significant difference in the number of CMBs among patients with varying lesion counts.
