| 目的 探究血清miR-425-5p、miR-501-3p、miR-140-5p与血管性痴呆(VaD)患者认知功能障碍(CD)严重程度及预后的相关性。方法 以2021年11月至2023年11月期间在河北北方学院附属第一医院就诊的124例VaD患者为研究组,选取同期健康体检者124例作为对照组。根据蒙特利尔认知评估量表得分,将VaD患者分为轻度组(18~26分,n=43)、中度组(10~17分,n=46)和重度组(<10分,n=35);根据预后情况,将VaD患者分为预后良好组(n=51)、预后中等组(n=42)和预后不良组(n=31)。采用实时荧光定量逆转录聚合酶链反应测定血清miR-425-5p、miR-501-3p、miR-140-5p水平;Pearson法分析VaD患者血清miR-425-5p、miR-501-3p、miR-140-5p水平与CD严重程度及预后的关系;通过受试者工作特征曲线分析上述3种血清的预测效能。结果 与对照组相比,研究组血清miR-501-3p、miR-140-5p表达上调(P<0.05),miR-425-5p表达下调(P<0.05)。随认知功能损害程度加剧,轻度、中度及重度组血清miR-501-3p、miR-140-5p水平呈递增趋势(P<0.05),血清miR-425-5p水平及MoCA评分呈递减趋势(P<0.05)。预后良好组、预后中等组、预后不良组患者血清miR-501-3p、miR-140-5p水平逐渐升高(P<0.05),血清miR-425-5p水平逐渐降低(P<0.05),且ADL评分逐渐降低(P<0.05)。VaD患者CD严重程度及预后与血清miR-425-5p水平呈正相关(r=0.435,0.405;P<0.001),与血清miR-501-3p水平呈负相关(r=-0.384,-0.513;P<0.001),与血清miR-140-5p水平呈负相关(r=-0.497,-0.460;P<0.001)。与血清miR-425-5p、miR-501-3p、miR-140-5p各自单独预测相比,3者联合预测VaD患者预后的曲线下面积最高(Z3者联合-miR-425-5p=4.340、P<0.001,Z3者联合-miR-501-3p=3.225、P=0.001,Z3者联合-miR-140-5p=3.452、P=0.001)。结论 VaD患者中血清miR-425-5p水平降低,miR-501-3p、miR-140-5p水平升高,与CD严重程度及预后相关,3者联合对VaD患者预后具有更好的预测价值。 |

| Objective To investigate the correlation between serum miR-425-5p, miR-501-3p, and miR-140-5p levels and the severity of cognitive dysfunction(CD) as well as prognosis in patients with vascular dementia(VaD). Methods The study group consisted of 124 patients with VaD who were treated at the First Affiliated Hospital of North Hebei University from November 2021 to November 2023. A control group of 124 healthy individuals undergoing routine physical examinations during the same period was also recruited. Based on the Montreal Cognitive Assessment scores, VaD patients were classified into the mild(18~26 points, n=43), moderate(10~17 points, n=46), and severe groups(<10 points, n=35). According to prognosis, VaD patients were categorized into the good prognosis(n=51), moderate prognosis(n=42), and poor prognosis(n=31) groups.Serum levels of miR-425-5p, miR-501-3p, and miR-140-5p were measured using real-time fluorescent quantitative reverse transcription polymerase chain reaction(qRT-PCR). Pearson's correlation analysis was used to assessed the relationship between serum miR-425-5p, miR-501-3p, and miR-140-5p levels and CD severity and prognosis in VaD patients. Receiver operating characteristic curve analysis was used to assess the predictive performance of these three serum markers. Results Compared with the control group, the study group showed upregulated expression of serum miR-501-3p and miR-140-5p(P<0.05) and downregulated expression of miR-425-5p(P<0.05). As the severity of cognitive impairment increased, serum levels of miR-501-3p and miR-140-5p showed an upward trend in the mild, moderate, and severe groups(P<0.05), while serum levels of miR-425-5p and MoCA scores showed a downward trend(P<0.05). Serum miR-501-3p and miR-140-5p levels progressively increased(P<0.05), serum miR-425-5p levels progressively decreased(P<0.05), and ADL scores progressively decreased(P<0.05) in patients with favorable, intermediate, and poor prognosis, respectively. In VaD patients, CD severity and prognosis were positively correlated with serum miR-425-5p levels(r=0.435,0.405;P<0.001), negatively correlated with serum miR-501-3p levels(r=-0.384,-0.513;P<0.001), and negatively correlated with serum miR-140-5p levels(r=-0.497,-0.460;P<0.001). Compared to the individual predictive performance of serum miR-425-5p, miR-501-3p, and miR-140-5p, the combined prediction of these three markers yielded the highest area under the curve for predicting prognosis in VaD patients(Zcombined-miR-425-5p=4.340, P<0.001;Zcombined-miR-501-3p=3.225, P=0.001; Zcombined-miR-140-5p=3.452, P=0.001). Conclusion In patients with vascular dementia(VaD), serum levels of miR-425-5p are reduced, while levels of miR-501-3p and miR-140-5p are elevated. These microRNA levels correlate with clinical dementia severity and prognosis. The combined assessment of all three microRNAs demonstrates superior predictive value for prognosis in VaD patients. |
